Prior authorization update for select DME

The following durable medical equipment (DME) codes will require prior authorization (PA) effective September 1, 2023.

Providers should review the list of codes that will require PA. For code-specific PA requirements, visit providers.anthem.com/ca.

PA requests can be submitted through Availity* at availity.com or by calling one of our Medi-Cal Customer Care Centers at 800-407-4627 (outside L.A. County) or 888-285-7801 (inside L.A. County).
