Required patient safety standards attestation

As a qualified health plan (QHP) issuer, we must ensure compliance with federal regulation 42 CFR 156.1110 when contracting with hospitals with more than 50 beds. This regulation mandates QHPs verify hospitals meet the specific patient safety standards detailed below:

  • The hospital must use a patient safety evaluation system as defined in 42 CFR 3.20 by:
    • Collecting and reporting data to patient safety organizations (PSOs)
    • Implementing a mechanism for comprehensive person‑centered hospital discharge
    • Improving care coordination and healthcare quality for each patient
  • If the hospital is not working with a PSO, the hospital must implement an evidence‑based initiative to improve healthcare quality, reduce preventable harm, prevent readmission, and improve care coordination.

To help us ensure compliance, we require participating hospitals attest to meeting these patient safety standards annually by completing the Patient Safety Standards for QHP Issuers form no later than June 30, 2025, and return it to your provider network manager/contractor.
