Provider satisfaction survey information

HealthKeepers, Inc. is conducting a survey to gauge your satisfaction with the Anthem HealthKeepers Plus provider services we offer. We will randomly select a number of providers to take part in this process. Selected providers will receive a questionnaire in the mail from SPH Analytics, an independent research firm.

 

What happens when providers receive the survey?

 

When you receive the survey, we would greatly appreciate your participation. The survey should take about 15 minutes to complete, and the answers collected will help improve the services we currently provide, and ultimately create a more collaborative partnership with the end goal of providing the highest quality care to our members, your patients.
