Beginning January 1, 2024, Empire became Anthem. This article, published under the former brand, now applies to Anthem.

In the October 2018 issue of our provider newsletter in the ‘Clinical Guideline Updates’, we incorrectly advised that the following guideline would be archived effective January 1, 2019.
