New reimbursement policy: Transitional Care Management - professional

Empire BlueCross BlueShield (Empire) has added value to the Transitional Care Management definition to include discharge from the emergency room to help prevent future emergency room encounters or hospital admissions.

 

As of April 27, 2022, Empire has implemented a new professional reimbursement policy: Transitional Care Management. When a member requires a transition to a community setting, the Transitional Care Management period begins upon the member’s discharge and continues for 29 days.
