New reimbursement policy: Modifier FB – Professional and Facility
Beginning January 1, 2024, Empire became Anthem. This article, published under the former brand, now applies to Anthem.
Beginning with dates of service on or after October 1, 2022, Empire BlueCross BlueShield (Empire) will implement a new professional and facility reimbursement policy titled, Modifier FB - Professional and Facility. Modifier FB should be appended to all devices, supplies, or drugs obtained at no cost to the provider. Services appended with modifier FB are not eligible for reimbursement.
In addition, modifier FB has been removed from the Modifier Rules - Professional policy.
