Guidelines for reporting timed units for Physical Medicine and Rehabilitation (Professional)

Beginning January 1, 2024, Empire became Anthem. This article, published under the former brand, now applies to Anthem.

Beginning with dates of service on or after April 1, 2021, Empire BlueCross BlueShield has updated our policy to reflect services must be reported with appropriate modifiers GN, GO and GP to identify therapy type.
