2021 Provider Manual now available
Beginning January 1, 2024, Empire became Anthem. This article, published under the former brand, now applies to Anthem.
We are pleased to announce that a new Empire Provider Manual, will be available on empireblue.com/provider on August 1, 2021. The provider manual replaces any prior version and it allows you to link directly to many important forms, policies, and tools on our website.
