Treasure Island Gap Plan ID Cards

Anthem Blue Cross and Blue Shield and our subsidiary company, HMO Nevada (Anthem) would like to remind providers that our Treasure Island account members will present two (2) ID cards, an Anthem ID card and a Morgan White Premium Saver ID card, at the time of service. Both Anthem and Morgan White should be billed for services rendered with Anthem as the primary insurer and Morgan White as the supplemental, or secondary, insurer.
