Submit preapproval requests for post‑acute care services for Medicare group retiree members to Carelon Post Acute Solutions

As of February 14, 2025, Carelon Post Acute Solutions reviews preapproval requests to admit our Medicare group retiree members for a concurrent stay in a skilled nursing facility, inpatient acute rehab facility, or long‑term acute care hospital. These requests should not be submitted through Availity Essentials.

To submit a preapproval request, upload clinical information, or check the status of a request, go to http://portal.mynexuscare.com. If you cannot access the website, call the Provider Call Center at 844‑411‑9622 from 7 a.m. to 7 p.m. CT, Monday through Friday.
