Program benefits:

The program, available at no extra cost, promotes autonomy, healthier living, and easy access to benefits. Key features include:

  • Helps patients set up medical, dental, or behavioral health homes to enhance their understanding of prevention and wellness.
  • Educates patients on new diagnoses and assists them in obtaining necessary medical equipment.
  • Supports patients in achieving healthier living through targeted smoking cessation programs.
  • Offers access to resources for high-risk obstetrics to reduce infant mortality and related risks.
  • Facilitates connections to ESL classes and cultural support resources to aid patient integration.

You can refer patients to us

Referrals can be sent by phone or fax. For phone referrals, call Provider Services at 866-408-6132 (Hoosier Healthwise), 844-533-1995 (Healthy Indiana Plan), 844-284-1798 (Hoosier Care Connect), or 833-569-4739 (Indiana PathWays for Aging), or the Care Management line at 866-902-1690. The Complex Care Management team is available Monday through Friday from 8 a.m. to 5 p.m. ET. For fax referrals, complete the referral form and fax to 855‑417‑1289. Patients can refer themselves

Patients and caregivers can refer themselves or family members by calling the Member Services number on their member ID card. They will be transferred to a team member based on their immediate need. Patients can opt‑in and out of the Complex Care Management program anytime.

Providers who are contracted with Anthem Blue Cross and Blue Shield to serve Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, and IN PathWays for Aging through an accountable care organization (ACO), participating medical group (PMG) or Independent Physician Association (IPA) are to follow guidelines and practices of the group. This includes but is not limited to authorization, covered benefits and services, and claims submittal.
