Attest to your practice’s data accuracy with Availity Essentials PDM

Starting January 1, 2026, care providers who have not attested to their data accuracy every 90 days via Availity Essentials’ provider data management (PDM) application or have not filed a claim in the past 12 months will be suppressed from Anthem provider directories. This suppression will directly affect members’ ability to choose in‑network care providers on their plans.

Availity Essentials’ PDM application is our exclusive method for verifying and initiating demographic change requests for all care providers. It is also the sole channel for submitting these requests and roster uploads via our roster automation standard template. Availity Essentials’ PDM helps create a more streamlined experience for members by ensuring their interactions with care providers and insurers are based on accurate and up‑to‑date information.

Care providers can still use the Provider Enrollment application in Availity Essentials to add new practitioners to existing groups.

Accessing the PDM application

Log on to https://Availity.com and select My Providers > Provider Data Management to begin the attestation process. If submitting a roster, find the TIN or business name you want to verify and update the information.

Before you select the TIN or business name, select the three‑bar menu option on the right side of the window, select Upload Rosters (see below), and follow the prompts.

Availity Essentials administrators are automatically granted access to PDM. An administrator may also grant additional staff access to PDM. To find your administrator, go to My Account Dashboard > My Account > Organization(s) > Administrator Information.
