Annual notice - Compliance with federal law, state law, and Anthem policies and procedures

This notice serves as an annual reminder that Anthem and its Providers agree to comply with applicable regulatory requirements relating to their obligations under the Provider Agreements, and maintain in effect all permits, licenses and governmental and board authorizations and approvals as necessary for business operations. Also, as pursuant to the Provider Agreements, contracted Providers agree to comply with Anthem's provider manuals, and applicable policies, programs and procedures established and implemented by Anthem with respect to the Networks in which Providers participate.
