Providers and facilities are required to review all member information received from Anthem Blue Cross (Anthem) and other providers to help ensure that misrouted protected health insurance (PHI) is not included. Misrouted PHI includes information about members that a provider or facility y is not currently treating. PHI can be misrouted to providers and facilities by mail, fax or email. Providers and facilities are required to immediately inform the sender and destroy any misrouted PHI and safeguard the PHI for as long as it is retained. In no event are providers or facilities permitted to misuse or re-disclose misrouted PHI.
