Friendly reminder — Use the correct forms and submission process for Behavioral Health prior authorizations

Use the correct forms

This is a friendly reminder that all providers are expected to submit Behavioral Health prior authorization requests to HealthKeepers, Inc. for members enrolled in the Anthem HealthKeepers Plus plan using the current corresponding DMAS-mandated forms that are posted on the DMAS website:

Submit via Availity Essentials or the correct fax number:

  • The preferred method of submission for authorization requests is online through Availity Authorizations. If you do not use this website or the correct fax line below, this will cause delays in processing time.
  • If unable to submit through Availity Essentials, please use the expedited fax line 844-445-6646 for all ARTS, Inpatient Psychiatric Hospitalization, Mental Health Intensive Outpatient Program, Mental Health Partial Hospitalization Program, Community Stabilization, Mobile Crisis Response, 23-hour Crisis Stabilization, and Residential Crisis Stabilization authorization requests.
  • If unable to submit through Availity Essentials, please use the standard fax line 844-445-6642 for all other Behavioral Health outpatient service authorization requests.
