Members with COPD eligible for health improvement program

Anthem Blue Cross and Blue Shield in Ohio and Propeller Health are collaborating to improve health outcomes for Medicare Advantage members with chronic obstructive pulmonary disease (COPD).

 

Medicare Advantage members with a history of at least one emergency room visit or hospitalization caused by COPD are eligible for this program.  Each participant in this free program will have individual contact with an Anthem RN specifically working with COPD. Anthem’s care management team will use Propeller’s online provider portal to help understand consumers’ progress in managing their COPD, providing more targeted, personalized care and treatment.
