Immunizations Covered by Vaccines for Children (VFC)

Anthem Blue Cross and Blue Shield (Anthem) providers who administer vaccines to children from 0 to 18 years of age should enroll in the Vaccines for Children program (VFC). Anthem will reimburse the administration fee for vaccines available through the VFC program only.

When billing multiple immunizations on a claim, providers should bill the administration code on multiple lines with one unit. Anthem does not require the SL modifier to be appended.
