Availity Essentials Patient360

Patient360 (P360) is a dashboard in Availity Payer Spaces that simplifies administration by coordinating access to member-centric patient information. P360 offers both clinical and case management data about a particular member’s record.

Below are the details P360 offers to our providers:

• Demographic information
• Gaps in care
• Care summaries
• Claims details
• Authorization details
• Pharmacy information
• Care management

Providers also have the option to include feedback for each gap in care that is listed on the patient’s Active Alerts under Member Summary.

There are two ways to access Patient360:

1. Log in to Availity.com, select Patient Registration > Eligibility & Benefits. Create a search for your patient and the button for P360 will be on the top menu bar of the results.
2. Log in to Availity.com, Select Payer Spaces from home page menu bar, then select the Anthem Blue Cross Blue Shield tile. Under the application tab select P360.

Note: To use Patient360, providers must have their user role assignment as Clinical > Patient360.
