Anthem will delegate care management to Carelon Health, Inc.

Effective May 1, 2025, we will delegate some care management (case management and disease management) services to Carelon Health, Inc.

This delegation of services will not impact the way we work with members or care providers. There will be no disruptions in service or impact on how you collaborate with care management staff. All contracts, policies, and procedures will remain unchanged. Additionally, all existing phone numbers, emails, websites, and portals will not change. Your patients’ current points of contact, engagement model, level of care, and service will not change. No action is needed from you.

Within our communications, you will see the following changes:

• A legal disclaimer — In your patients’ plan communication, you will see a footnote referencing the fact that Carelon Health, Inc. is now providing these care management services on behalf of Anthem.
• Signature of care management/staff healthcare professional — While the team providing services will not change, you will see that their signatures may change to reflect Carelon Health, Inc.
