Anthem MA Ohio Field Based Case Management Program

Anthem Medicare Advantage in Ohio has a new field-based case management program in Lucas County. Nurses visit our Medicare Advantage members while they are in the hospital, skilled nursing facility and at home.  The purpose of the program is to help the member overcome barriers to necessary medical care (i.e. physician appointments, medications, transportation, etc).
