Healthy Blood Pressure Recheck initiative: CBP
We remain committed to promoting the Healthy Blood Pressure Recheck as part of our efforts to collaborate with care providers and members for controlling high blood pressure (BP).
Anxiety during physician visits can lead to temporary BP increases, resulting in inaccurate readings and potential overtreatment. This initiative encourages rechecking BP readings higher than 139/89 mm Hg at the end of the patient’s appointment. If the reading remains elevated, schedule a follow‑up. Please follow these simple steps to incorporate this initiative into your daily workflow:
- Measure the patient’s BP when they first enter the exam room.
- If the reading is above 139/89 mm Hg, take another reading at the end of the appointment.
- If the BP is still elevated after the second reading, schedule a recheck within the next two weeks.
Record precise readings to help evaluate whether the patient’s medications or lifestyle changes are effectively managing their hypertension.
Submit all BP readings via CPT® Category II codes.

When multiple BP readings are taken on the same day, select the lowest systolic and diastolic readings as the representative values for that date.
Record the member's most recent BP reading during telehealth visits. BP readings taken by the member using a digital monitor and documented in the member’s medical record are eligible for use in reporting.
What doesn’t count?
Do not include BP readings:
- Taken during an acute inpatient stay or an emergency department visit.
- Taken on the same day as a diagnostic test or diagnostic/therapeutic procedure that requires a change in diet or medication on or one day before the day of the test or procedure, except for fasting blood tests.
- Taken by the member using a non‑digital device, such as a manual BP cuff and a stethoscope.
