IUOE 513 Welfare Fund moves to Anthem Medicare Preferred (PPO) medical plan
Effective August 1, 2020, the International Union of Operating Engineers (IUOE) 513 Welfare Fund will offer a PPO plan for Medicare members enrolled in Anthem Blue Cross and Blue Shield (Anthem).
Anthem will provide medical benefits for the retirees of IUOE 513 Welfare Fund with Medicare Parts A and B through Anthem’s Local Preferred Provider Organization (LPPO) product. This plan includes the National Access Plus benefit, which allows retirees to receive services from any provider, as long as the provider is eligible to receive payments from Medicare.
In addition, IUOE 513 Welfare Fund retirees pay no cost share for both in-network and out-of-network services. Members’ cost share does not change, regardless of services performed locally or nationwide, by doctors or hospitals, or in network or out of network.
The Medicare Advantage plan offers the same hospital and medical benefits as original Medicare, as well as an annual routine physical exam, hearing, vision, chiropractic care, LiveHealth Online* and SilverSneakers®.* The prefix on IUOE 513 Welfare Fund member ID cards will be ZMX. The cards will also show the National Access Plus icon.
Providers may submit claims electronically using their Anthem electronic payer ID or submit a UB-04 or CMS-1500 form to Anthem. Detailed prior authorization requirements also are available to contracted providers by accessing the Provider Self-Service Tool at availity.com.* Claims should not be filed with original Medicare.
