Reimbursement policy update: Modifier FB — Professional and Facility

Beginning with dates of service on or after January 1, 2025, Anthem will expand the current Modifier FB — Professional and Facility reimbursement policy to apply to facility providers.

As a reminder, this policy requires modifier FB to be appended to all devices, supplies, or drugs obtained at no cost to the provider. Services appended with modifier FB are not eligible for reimbursement.
