Important Crib and Car Seat Program update

Anthem Blue Cross and Blue Shield Medicaid (Anthem) recently launched a new and exciting value-added benefits program. As part of this program, we've made it easier for members to access their benefits. One of the changes improves the Crib and Car Seat Program. Members no longer require a faxed referral from their doctor.

 

Instead, members must now either call Anthem Customer Service at 855-690-7784 (TTY 711), Monday through Friday from 7 a.m. to 7 p.m., or log in to their Anthem account at mediproviders.anthem.com/ky and select Benefit Reward Hub to order their crib or car seat.
