Claim rejection notice
Effective August 1, 2024, Anthem will reject claims that contain national provider identifiers (NPIs) not enrolled with the Kentucky Department of Medicaid Services (DMS). Rejected claims will not be accepted by Anthem and will not be assigned a claim number. Providers should submit claims with attending, billing, ordering, prescribing, rendering, and referring NPIs that correspond to the data on the Master Provider List (MPL).
To correct rejected claims:
- Confirm that the attending, billing, and rendering on your claim include the NPI and taxonomy.
- If your attending, billing, and rendering are correctly reported on the claim, contact the Kentucky Department of Medicaid Services Enrollment department at 877-838-5085 to verify your information is valid and matches the Master Provider List. Verify the following information:
- Attending, billing, and rendering NPI with effective and term dates
- Attending, billing, and rendering taxonomy with effective and term dates
- Remit address with city, state, ZIP code + 4
