Anthem will no longer perform precertification requests for General Motors (GM) members as of 12/01/2018 for:

• Acute Inpatient Medical Hospital
• Acute Inpatient Rehabilitation
• Skilled Nursing
• Long Term Acute Care
• AIM Radiology and
• Integrated Health Management (360 Health)

 

To request precertifications, facilities are encouraged to utilize the self-service tools available through the Availity portal or by using the available fax forms located on the BCBSM.com website.

2019 Facility Precertification and Integrated Health Management Changes for General Motors (GM) Members

 

• Anthem Precertification approvals are valid for dates of service in calendar year 2018.
• Anthem Integrated Health Management engagement will continue through DOS 11/30/2018.

 

AIM Radiology for GM, Fiat Chrysler & Delphi/APTIV

 

• Anthem AIM Radiology approvals are valid for dates of service in calendar year 2018.
