Provider Satisfaction Survey reminder

We will be sending out the Provider Satisfaction Survey by email and mail in July 2025 and are reminding care providers to complete it as soon as possible. Our Provider Satisfaction Survey is an important tool, enabling us to make evaluations that will help us improve our processes and provide exceptional service to our care providers, which is one of our strongest commitments.

Indiana PathWays for Aging has contracted with the Center for the Study of Services (CSS), an independent company, to administer the survey on our behalf. If you have any questions about the online survey, please email CSS at ProviderSurvey@cssresearch.org.
