Indiana’s ACA transformation — Pathway Essentials becomes Anthem Essential Network

Indiana is renaming its Affordable Care Act (ACA) plan sold via the Individual marketplace, on and off exchange. Effective January 1, 2025, the Pathway Essentials network will be renamed Anthem Essential Network. Members will receive new ID cards for 2025 that will include the new network name and the metal level for their policy, such as Anthem Bronze Essential or Anthem Gold Essential. The Anthem Essential network will remain a narrow network and the alpha prefixes (listed below) on the ID number will not change:

• Anthem Essential Network HMO Prefixes: E7E or K5N
• Anthem Essential Network POS Prefixes: S8P or X6K
