Claim updates and announcements for Indiana PathWays for Aging care providers

  • Z68 claim denials — Anthem has identified an error for claims being denied with code Z68 and is reprocessing these denials for payment to remove the Z68 error. We are in the final stages of reprocessing claims that were initially denied for Z68, with a permanent fix put into place since February 5, 2025. New claims submitted should not be affected by the Z68 denials. Please note that claims submitted as duplicates will still be denied.
  • Providers billing for Assisted Living Services with CPT® code T2031 should ensure to bill for the appropriate units, depending on whether you are billing for the monthly rates versus the daily rate:

Use Availity Essentials to access and review claim file status reports:

  • Providers can use the EDI Reporting Preference Application to set up and receive claim status reports for claims submitted through Availity, including reports for rejected claims. Log in to your Availity profile and select Claims and Payments > EDI Reporting Preferences. Then select the claims tab to get started.
  • Access your report files through the Send and Receive Files Folder in Availity Essentials by selecting claims and payments > Send and Receive Files Folder > Received Files.
    • Note: You will need to have the proper Availity role access assigned to your username to access either of these tools. Reach out to an admin on your Availity account for assistance with getting the proper access.
