Important Notice for the Anthem Dental Care Network primary care dentist

Dental Care Network providers: This is a reminder for all doctors to submit encounter forms. Ensure all required fields are populated or there may be a delay in encounter payments. The Encounter Form has been updated to include additional required fields:

• Office address (treating location)
• Office tax ID
• Dentist individual NPI

Additional reminder: For the subscriber ID, make sure to use the alternate ID number only.

Note: ADA claim forms sent in with encounter data must have Encounter written on them. Ensure all required fields are populated.
