Streamline your payments submission for Anthem's dental care network primary care dentist

This applies to dentists in New York and New Jersey.

Dental Care Network care providers: All doctors should submit encounter forms. Ensure all required fields are populated, or there may be a delay in encounter payments. The encounter form has been updated to include additional required fields:

  • Office address (treating location)
  • Office tax ID
  • Dentist individual NPI

For the subscriber ID, make sure to use the alternate ID number only.
