Connecticut groups move to our Medicare Advantage plan

We are pleased to announce Medicare Advantage plan membership growth in the state of Connecticut beginning January 1, 2025. Several groups in Connecticut will offer our Medicare Preferred Provider Organization (PPO) plan to their retirees.

These groups include:

  • Connecticut College
  • Town and Board of Education of Simsbury
  • Town of Stratford
  • West Shore Fire District
  • IUOE Local 478
  • Town of Watertown
  • LL Global, Inc.
  • Montville Public Schools
  • City of Norwich
  • Town of Hamden
  • City of New London
  • University of New Haven
  • Hartford Public Schools
  • Brookfield Town and Board of Education

We will provide medical benefits for new Medicare Advantage group retirees through our PPO product, which includes the National Access Plus benefit. The PPO plan allows members to receive services from any eligible provider eligible to receive Medicare payments.

Medicare Advantage group members’ copay or coinsurance percentage will be the same whether their provider is in or out of network. Locally or nationwide, whether for doctors or hospitals, member share‑of-cost (SOC) does not change.

Non‑contracted providers may continue treating Medicare Advantage group members and will be reimbursed 100% of Medicare’s allowed amount for covered services.

The Medicare Advantage plan offers the same hospital and medical benefits that Medicare covers while covering additional benefits that Medicare does not, such as LiveHealth Online® and SilverSneakers®.

The prefix on the Medicare Advantage ID cards is ZDX.

Contracted providers can also access detailed prior authorization requirements by accessing the Provider Self‑ServiceTool on Availity Essentials at https://Availity.com.

Providers will follow their normal claim filing procedures for Medicare Advantage group member claims.
