Enhance patient care with our Complex Case Management program
Managing illness can be daunting for patients. They can struggle to understand test results, find resources for treatment, and know who to contact about their concerns. We’ll help your patients with transitions across care levels to be better prepared and informed about healthcare decisions and goals. Refer your patients by submitting a Case Management Referral Form by email or fax:

Patients can refer themselves
Patients and caregivers can refer themselves or family members by calling the Member Services number on their member ID card. They will be transferred to a team member based on their immediate need. Patients can opt in and out of the Complex Case Management program anytime.
Physicians can refer their patients by submitting a Case Management Referral Form by email or fax.

Our case managers are part of a team of clinicians and resource professionals who support providers, patients, families, and caregivers. We educate patients, help them understand their illnesses, and empower them by increasing their self‑management skills. Case management can help them learn about care choices to ensure access to quality and efficient healthcare.
